Medicaid Waiver Services

Home and community-based services (HCBS) provide opportunities for Medicaid beneficiaries to receive services in their own home or community rather than institutions or other isolated settings.

M4A’s Elderly & Disabled (E&D) Waiver Program serves a variety of targeted populations, such as people with intellectual or developmental
disabilities, physical disabilities, and/or mental illnesses.

Available services to those who qualify include case management, homemaker services, personal care, respite care (skilled and unskilled), adult day health, adult companion services, and home-delivered meals.

The Plan of Care and Case Management services are based on the individual client’s needs.
